Chikungunya or Dengue? The Tests That Tell Them Apart

In Gurugram's post-monsoon weeks, both dengue and chikungunya circulate together — spread by the same daytime-biting *Aedes* mosquito. The treatment differs, so the diagnosis matters.

Clues Before the Test

Neither is reliably diagnosed by symptoms alone.

The Right Test at the Right Time

• Day 1–5 of fever: Dengue NS1 antigen, and chikungunya RT-PCR where available
• After day 5: Dengue IgM, chikungunya IgM
• Always: CBC with platelet count, repeated every 24–48 hours while fever lasts
• Consider malaria antigen/smear and typhoid tests in the same visit — a fever panel is more efficient than one test at a time

An IgM test on day 2 is usually negative and gives false reassurance. Test by the calendar of your fever, not by convenience.

Warning Signs — Go to Hospital

Bleeding gums or nose, black stools, persistent vomiting, severe abdominal pain, restlessness, cold clammy skin, or a drop in urine output.

Care While Recovering

Paracetamol only — avoid ibuprofen, aspirin and other NSAIDs until dengue is ruled out. Fluids through the day. Recheck platelets as advised.
