Typhoid Test: Widal vs Typhidot vs Blood Culture — Which One Is Reliable and When?

Quick answer: The most reliable typhoid test is a blood culture, ideally done in the first week of fever and before antibiotics are started. The Widal test detects antibodies and is often falsely positive in India because of past infection or vaccination, so a single Widal result should not be used alone to diagnose typhoid. Typhidot (a rapid antibody test) is quicker but also has false positives and negatives. Doctors usually combine your symptoms, a CBC and a blood culture — with Widal or Typhidot as supporting tests.

Typhoid (enteric fever) is caused by Salmonella Typhi and Paratyphi bacteria, spread through contaminated food and water. In Gurugram, cases peak in the monsoon and post-monsoon months but occur year-round, particularly from street food, unfiltered water and poorly stored cut fruit.

The problem we see most often in our lab isn't missed typhoid — it's over-diagnosed typhoid. Many patients arrive having already taken several days of antibiotics on the basis of a single positive Widal report.

Symptoms That Should Make You Think of Typhoid

    • Fever that rises step-by-step over several days, often highest in the evening
    • Headache and body ache
    • Loss of appetite, abdominal pain, constipation or diarrhoea
    • Weakness and tiredness out of proportion to the fever
    • A coated tongue; in some people a relatively slow pulse despite fever

Fever lasting more than three days deserves medical evaluation and testing — see our guide to fever panel testing.

    The Three Typhoid Tests Compared

Blood Culture — The Gold Standard

    • How it works: a blood sample is incubated to grow the actual bacteria
    • Best time: first week of fever, before antibiotics
    • Advantages: confirms the diagnosis and gives an antibiotic sensitivity report — increasingly important because of drug-resistant typhoid strains
    • Limitations: results take 2–5 days; sensitivity falls sharply once antibiotics have been started
    • Tip: a larger blood volume improves detection; follow the lab's instructions

Aarvak's microbiology laboratory performs blood culture and sensitivity testing.

Widal Test — Widely Used, Widely Misread

    • How it works: detects antibodies (called O and H agglutinins) against typhoid bacteria
    • Best time: antibodies usually appear from the end of the first week; testing earlier often gives a negative result even with typhoid
    • Why false positives happen: past typhoid, typhoid vaccination, other infections and the high background level of antibodies in people living in India
    • How to read it better: a rising titre on two samples 7–10 days apart is more meaningful than one positive result. Low titres like 1:80 are common in healthy people in endemic areas

    Typhidot (Rapid IgM/IgG Antibody Test)

    • How it works: rapid card test detecting IgM (recent infection) and IgG antibodies
    • Best time: after the first few days of fever
    • Advantages: quick results, inexpensive
    • Limitations: both false positives and false negatives occur; it cannot give antibiotic sensitivity

    Other Tests That Help

    • CBC: in typhoid, the white cell count is often normal or low rather than high; platelets may be mildly low. See understanding CBC
    • Liver function test: mild rise in liver enzymes is common
    • Stool culture: more useful in the second and third weeks, and for checking carriers — important for food handlers
    • Dengue and malaria tests: to rule out other causes of fever in the same season

Why Starting Antibiotics Before Testing Is a Problem

Taking antibiotics "just in case" before a blood culture:

    • Lowers the chance the culture will detect the bacteria
    • Masks the true cause of the fever
    • Contributes to antibiotic resistance, which is already a serious concern with typhoid in South Asia

If your doctor suspects typhoid, ask whether a blood culture can be collected before the first antibiotic dose.

    Prevention

    • Drink boiled, filtered or sealed bottled water
    • Avoid raw salads and cut fruit from street vendors
    • Wash hands with soap before eating and after using the toilet
    • Consider typhoid vaccination — ask your doctor, especially before travel or if you work in food handling

    Frequently Asked Questions

Which test is best for typhoid?

Blood culture is the most reliable test for typhoid, especially when done in the first week of fever before antibiotics. It also shows which antibiotics will work.

Can the Widal test be wrong?

Yes. Widal often gives false positive results in India due to past infection, vaccination or cross-reacting infections, and can be negative early in the illness. It should be interpreted together with symptoms and other tests.

On which day of fever should the typhoid test be done?

Blood culture is best in the first week of fever. Widal and Typhidot antibody tests are more likely to be positive after the first five to seven days.

    Is Typhidot better than Widal?

Typhidot gives faster results and detects IgM antibodies of recent infection, but it also has false positives and negatives. Neither replaces blood culture.

How long does a blood culture report take?

Preliminary results can be available in 2–3 days, and a final report with antibiotic sensitivity usually takes 3–5 days.

Can I get a typhoid test at home in Gurugram?

Yes. Aarvak can collect blood samples for typhoid testing, including blood culture, at home across Sector 67, Sohna Road and nearby areas. Call +91 92663 33711.
